循環器内科、腎臓内科（専修医）

 

1 診療科の特徴

　循環器科は、循環器・腎臓内科として循環器疾患の臨床、教育、研究を担当している。病床数は腎臓内科と併せて一般病棟32床、ICU 7床、循環器科指導医は7名、専門修練医（後期研修医に該当）５名、研修医２－３名で、全員が協力、分担し診療を行っている。希望により腎臓グループや救急部へのローテーションが簡便に行える。
　循環器疾患全般の診療を行っており、非侵襲的な診断、動脈硬化危険因子の管理、心不全や不整脈の薬物・非薬物療法、待機的な血管インターベンションまで広く行っている。循環器疾患は、急性増悪をきたし緊急処置を要する患者が多く、急性心筋梗塞、急性心不全、不整脈などの急患も多く、24時間体制で救急診療を行っている。
　心臓・大血管・末梢血管のエコー診断を幅広く行い、動脈硬化危険因子の管理を積極的に行い、重症例にたいしてはＬＤＬアフェレーシスも行っている。虚血性心疾患（狭心症や心筋梗塞）に対しては、心臓カテーテル検査、経皮的冠動脈形成術（PCI /ステント留置術）を安全に行っており、成功率９４－９８％で、薬剤溶出性ステントの導入により再狭窄率も５％未満に低下している。また、不整脈患者に対しては、頻脈性不整脈患者に対するカテーテルアブレーション（経皮的心筋焼灼術）や植え込み型除細動器（ICD）、徐脈性不整脈患者に対する恒久的ペースメーカ植え込み術を行っている。心機能を保つ上で有利な右室流出路ペーシング（右室心尖部ペーシングに比べて）は日本においては当院から始まった。ペースメーカの電池交換は外来での日帰り手術を行っている。ICD植え込みは九州地区大学病院では第２位の症例数があり、ペースメーカ植え込み術は九州地区大学病院で最も症例数が多い。心機能が低下した心不全症例に対する心臓再同期療法（CRT-D）も行っている。神経調節性失神のチルト診断および治療においても当科は日本のパイオニアである。
　また、心臓血管外科との連携も緊密にしており、定期的に術前・術後カンファレンスを開き、冠動脈３枝病変や弁膜症といった外科手術症例も増加している。

循環器グループ後期研修の目標

I.一般目標

患者の立場に立った医療の行える、優れた臨床循環器内科専門医師を養成する。
循環器疾患の病態を理解し、適切な診断と治療を行なう知識及び手技を習得する。

 

II. 臨床研修の目標

【一般目標】

1. バランスのとれた循環器内科医として必要な基本的知識,診断・治療技術を実践できる。
2.  プライマリーケアに必要な臨床的能力を身につけるとともに、救急診療にも対応できる。
3.  患者およびその家族に病状をインフォームドコンセントが出来る。 また、それに則り、安全、 正確に必要な検査及び治療を実施できる。
4.  EBM に基づいた治療ができる。
5.  チーム医療を理解し、実践できる。

2. 心・血管の解剖、血行動態、電気生理を理解し、診断・治療計画を立案できる。

《腎臓グループ》

　腎臓内科は、循環器・腎臓内科として腎疾患の臨床、教育、研究を担当している。病床数は循環器科と併せて一般病棟32床、ICU 7床、腎臓内科指導医は４名、専門修練医（後期研修医に該当）２名、研修医２－３名で、全員が協力、分担し診療を行っている。希望により循環器グループや救急部へのローテーションが簡便に行なえる。
　腎疾患全般の診療を行っており、一次性および膠原病などを中心とした二次性の糸球体／間質性腎炎に対してエコー下腎生検等の検査を施行している。組織診断を行ない臨床診断／症状をふまえて治療している。また、慢性および急性腎不全の精査加療を行なっている。腎不全の原疾患を検査するとともに血液透析および腹膜透析の導入や内シャント作成術などのバスキュラーアクセスの作成や腹膜透析のテンコフカテーテル留置術を行なっている。
　腎センターには１４台の血液透析装置および３台の血漿交換装置があり、他科に入院している維持透析患者も含めて血液浄化を行なっている。透析導入の症例数は九州地区大学病院の中でトップレベルを保っている。膠原病などの自己免疫疾患のほかTTP・薬物中毒や肝不全などの疾患に対しての吸着療法や血漿交換療法などの血液浄化法を行なっている。急性腎不全を含め緊急の血液浄化や徐水などが必要な事態も多く、24時間体制で救急診療を行っている

5 当診療科での修練のメリット

【循環器グループ】

　循環器疾患の適切な診断、治療計画を作成できるようにするとともに、基本的な循環器救急に対応できるようにすることを目標とする。腎臓診療や救急部とのローテーションが円滑に行え幅広く勉強できる。
　主として病棟で指導医のもと主治医として患者を受け持ち、循環器疾患に関する診療技術と知識を学ぶ。心エコー検査や心臓核医学検査、64列MDCT(平成19年導入予定)、MRIをはじめとする非侵襲的な検査法から、心臓カテーテル検査、電気生理学的検査、ペースメーカ・ICD植え込み術の技術、知識の習得が可能である。平成９年からCPA患者の受け入れを開始し、平成16年には救急病院の指定を受けたことから、当院救急部からの依頼による急性心不全、急性心筋梗塞、心室細動・頻拍など循環器疾患患者も増加している。近年腎機能低下や透析症例の冠動脈疾患患者が増加しており、当科では腎臓内科と同じ診療科であることから、このような合併症を持つ循環器疾患症例の経験を多く積むことができる。
　当科では、研修後に臨床系の大学院教育を行っており、後期研修の後に循環器診療を続けながら大学院に入学し、臨床研究を行い学位取得も可能である。また、基礎医学系の大学院教育も行っており基礎研究による学位取得も可能である。さらに、北九州内外の関連病院で循環器・腎臓・救急・一般内科の臨床を継続することも可能である。

【腎臓グループ】

　慢性および急性腎炎（糸球体／間質性）の精査／加療について実際携わり、修得していく。一次性および膠原病などを中心とした二次性の糸球体／間質性腎炎に対してエコー下腎生検等の手技を実際に行ない、腎生検カンファにおいて組織診断を修得していくとともに、実際に治療に携わっていく。
　慢性および急性腎不全の加療を行う。血液透析および腹膜透析の導入や内シャント作成術などのバスキュラーアクセスの作成や腹膜透析のテンコフカテーテル留置術を修得するとともに、水曜日の腎センターカンファおよび金曜日の腎グループカンファに参加し腎不全患者の加療を行う。また、腎センターには１４台の血液透析装置および３台の血漿交換装置があり、腎センターカンファに参加するとともに、他科に入院している維持透析患者の血液浄化に携わっていく。さらに、膠原病などの自己免疫疾患のほかTTP・薬物中毒や肝不全などの疾患に対しての吸着療法や血漿交換療法をおこなっていく。二次性高血圧症の精査など行ない、高血圧症患者の検査／加療をおこなっていく。
　当科では、研修後に臨床系の大学院教育を行っており、後期研修の後に腎臓診療を続けながら大学院に入学し、臨床研究を行い学位取得も可能である。また、基礎医学系の大学院教育も行っており基礎研究による学位取得も可能である。さらに、北九州内外の関連病院で腎臓・循環器・救急・一般内科の臨床を継続することも可能である。
